TOYOTA BLUEGRASS MIRACLE LEAGUE

The Toyota Bluegrass Miracle League is open to children ages 5-19 with a special need, mental or physical. TBML plays both a spring and a fall season.  Spring registration typically begins in February with the season running from April until early June.  Fall registration begins at the end of the spring season with its season running from September until mid-October.  Please register your child to play as soon as possible during the registration periods.

PLEASE PRINT AND COMPLETE BOTH FORMS BELOW (INCLUDING OBTAINING A PHYSICIAN'S SIGNATURE ON THE MEDICAL HISTORY STATEMENT) AND MAIL THE ORIGINAL FORMS TO:
